Terms & Conditions

1. E-claim submissions allowed for Outpatient, Dental, and Vision Benefits, and for medical expenses to be applied to deductible only.
2. Maximum claim amount is Rp. 10.000.000 per transaction/ participant.
3. Original documents are not required.
4. Acceptable format for files are .jpg,. ipg, .png, PDF
5. Maximum 5 Mb per file and maximum 25 Mb per transaction.
6. Photo is in portrait format.
7. E-claim submissions accepted between 08:00 - 16:00 Monday to Friday. If submitted after 16:00, it will be recorded the next work day.
8. The Insurer has the right to request for the original documents for the claim submission if necessary for the purposes of claim analysis or investigation.
9. Should any special policy provisions (Terms & Conditions) regarding the method of submitting claims are approved and issued, these special policy provisions will apply.
